This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our duties
Impact Transportation, LLC is required by law to protect the privacy of your Protected Health Information (PHI), provide you this notice of our legal duties and privacy practices, and follow the terms of this notice.
How we use and disclose your PHI
- For treatment and care coordination: We share trip details with the medical facility you are traveling to or from when needed for your care.
- For payment: We share trip records with Medicaid, TennCare brokers (such as Verida), or other payers to bill for your transportation.
- For health care operations: We use trip and GPS data internally for quality assurance, training, and audit response.
- As required by law: We may disclose information to regulators, courts, or public health authorities when required by law.
Your rights
- Access: You have the right to inspect and obtain a copy of your PHI we hold.
- Amendment: You may request that we correct PHI you believe is inaccurate.
- Accounting of disclosures: You may request a list of certain disclosures we have made.
- Restrictions: You may request that we restrict how we use or disclose your PHI; we will consider but cannot guarantee all restriction requests.
- Confidential communication: You may request that we contact you in a specific way (for example, by phone only, not email).
- Paper copy: You may request a paper copy of this notice at any time.
Driver and dispatch training
All Impact drivers and dispatch staff receive HIPAA privacy and security training before handling member rides. Discussion of member information is limited to what is necessary to complete the trip.
